Strengthening Health Services for the care of people with High Blood Pressure and Diabetes in the Province of Corrientes

Summary

The purpose of this proposal is to develop a comprehensive program that strengthens health services for the care of people with high blood pressure and diabetes in the province of Corrientes. The program's objectives are: 1) Implement a training program for primary health care teams for prevention, clinical management of cardiovascular risk factors (high blood pressure and diabetes), and the implementation of educational interventions aimed at reduce cardiovascular events and complications derived from the disease; 2) Implement an information and communication system that includes a Registry of people with Diabetes and/or Hypertension that allows the population to be identified and monitored and; 3) Implement a strategy based on Mobile Health to improve the assistance and monitoring of the population in primary health care centers (CAPS) or hospitals.
