Objectives:

The purpose of this study is to find out if a comprehensive plan that includes home health education, blood pressure self-monitoring, and health-related text messaging can reduce blood pressure levels in patients with hypertension and their families, compared to the usual care provided by their primary care physician.

• Net difference in SBP and DBP between the baseline determination and that recorded at 18 months in the intervention versus control group.
Secondary Objectives
• Proportion of controlled hypertensive patients (BP < 140/90 mmHg or < 130/80 mmHg if they have diabetes or chronic renal failure).
• Net difference in SBP and DBP between the baseline determination and that recorded at 18 months between hypertensive versus non-hypertensive patients.
• Cost effectiveness of the intervention.
• Net difference in weight and waist circumference.
• Changes in physical activity levels and diet.

Summary

Randomized intervention at the CAPS level of the public primary care network in collaboration with the Remediar+Networks Program of the Ministry of Health. A multifaceted intervention will be carried out for the prevention and control of HTN in hypertensive patients and their families by PC teams and health agents.
